過去對於癌症的研究多集中在腫瘤本身,因此除了手術摘除腫瘤治外,傳統的治療是以化學治療為主抑制癌細胞的增生,或是以標靶治療阻斷癌細胞表現之特殊的生存路徑。然而,經過幾十年的努力,癌症的死亡率仍然高居於我們國人的十大死因之首。早在1889Stephen Paget就提出種子與土壤的假說來解釋腫瘤轉移的現象。種子就是擴散的癌細胞,必須有適當的土壤才會萌芽、生長。這個假說經過一百多年之後重新被醫學界提出來討論,主因是近年來的研究顯示,腫瘤的生存與發展與周圍微環境(土壤)中的其他細胞(如纖維細胞、內皮細胞、免疫細胞)及、結締組織及細胞外基質等有很密切的關係。腫瘤與周圍的非癌細胞之間可透過不同細胞分泌出來的物質,或細胞表面的膜蛋白進行溝通。這些溝通,對於腫瘤的形成與發展有的是正面的,有的是負面的。因此,當我們明確的瞭解腫瘤微環境的活動對於腫瘤的作用,我們可以針對癌細胞與周圍環境的溝通管道,研發新的治療方針,改善腫瘤的微環境,降低腫瘤的惡化程度及轉移能力。

        為了研究腫瘤微環境,我們實驗室以一位女性肺腺癌病人的正常肺組織及肺癌組織進行精簡雜交反應,建立了兩個精簡雜交基因庫-分別含有抑癌基因及致癌基因的基因庫。由於我們對於腫瘤微環境的興趣,我們選擇了分泌型蛋白及膜蛋白的基因進行研究。

  1. 本實驗室的研究主題包含:
  1. Slit2蛋白(抑癌蛋白): Slit2是一個分泌型蛋白,具有抑制神經軸突的移動、白血球的趨化作用,與肌肉的形成及肺臟、腎臟的發育有關係。我們實驗室發現Slit2具有兩個剪接變異型的表現: Slit2-WTSlit2-DE15Slit2-DE15可以抑制肺癌細胞的生長及侵犯能力,但是Slit2-WT卻只能抑制侵犯能力。我們發現肺癌病人的正常肺組織表現的主要是Slit2-WT,而高比例的氣胸病人有表現較高的Slit2-DE15。因此,我們想了解不同的剪接變異型是否與肺癌的發生有關,是否可以促進Slit2-DE15的表現來抑制肺癌細胞的生長及轉移。除此之外,我們也發現Slit2-WTSlit2-DE15對於血管的正常化有不同的功能。Slit2-WTSlit2-DE15都具有抑制肺癌所誘導的血管通透性,但是只有Slit2-DE15具有讓血管正常化的能力。最近的研究顯示,腫瘤附近的血管正常化可以增加化療藥物及放射線治療的療效。我們的研究顯示Slit2-DE15不僅可抑制肺癌細胞的生長、侵犯並可以抑制血管的通透性及增加血管的正常化,因此Slit2具有潛力發展成為抗肺癌的生物藥物。

我們更進一步的從Slit2的蛋白質中鑑定出了8個胺基酸的胜肽具有抑制肺癌細胞的生長及侵襲能力、抑制小鼠體內腫瘤的生長及轉移能力、並可以降低化療藥物順鉑的毒性。我們致力於修飾這個胜肽,並研究這個胜肽是透過什麼受體及訊息傳遞路徑抑制癌細胞的生長及侵犯能力。肺癌存活率低是因為癌細胞早期的轉移率高,由於此胜肽可以同時抑制癌細胞的生長及侵襲能力,我們希望能將此胜肽發展成為安全的支持性療法的抗癌藥物,讓肺癌成為一種可以有效控制的慢性疾病。

  1. OPN (致癌蛋白): OPN也是分泌型蛋白,在免疫調節上扮演著重要的腳色。在許多的癌症中OPN有過度表現的現象,當血液中OPN的濃度高時,肺癌病人的預後不好。OPN也有三種剪接變異性的表現- OPN-a, OPN-bOPN-c。我們發現OPN-a在肺癌組織及肺癌細胞中表現最高,但是OPN-a並不是向過去的報告顯示只會促進肺癌細胞的生長及侵犯。我們發現OPN-a在濃度低時會透過integrin b3的受體抑制細胞的生長,但是如果integrin b3受體表現低時,OPN-a就會結合到另一個受體-CD44,而促進細胞的生長。因此,肺癌細胞的生長就會受到環境中OPN-a的濃度及肺癌細胞株表面所表現之integrin b3 CD44受體的調控。我們的研究對於發展OPN, integrin b3相關的標靶治療提出很重要的訊息。由於,目前有許多藥物是針對integrin beta family進行抑制,但是對於OPN濃度高的病人,抑制integrin b3可能會促進細胞的生長,而是應該考慮同時抑制OPNCD44較佳。除此之外,我們也想要了解OPN對於腫瘤微環境中的其他非癌細胞有何影響,以及什麼機制讓OPN在癌細胞中會高度表現。這些問題的瞭解將有助於我們發展對抗肺癌的新方針。

  1. 本研究室的研究模式:
  1. 以分子生物學為基礎,進行基因突變。分子生物研究是我們實驗室一個基本及重要的訓練。學生學習如何將感興趣的基因構築出來、進行基因突變,學習如何檢測基因的表現等相關問題。
  2. 以體外細胞實驗分析突變基因的功能。將所構築的基因轉殖至細胞株中,分析基因的功能、訊息傳遞路徑及突變基因對功能有何影響。
  3. 以動物實驗證明細胞實驗所觀察的抑癌功能。在體外細胞實驗得到顯著的抑制肺癌細胞生長、侵犯或影響血管新生的作用之後,我們會進入動物實驗或雞胚胎實驗證實細胞實驗的結果。
